Life Long Care is a program designed to respond to the life long care needs of older Aboriginal people who may have limited mobility related to a persistent medical condition or physical disability.

Introduction

Following consultations with off-reserve Aboriginal communities, the Ontario Federation of Indian Friendship Centres (OFIFC) entered into an agreement with the Ministry of Health to provide life long care services through local Friendship Centres. The Odawa Native Friendship Centre (ONFC) launched its program in 1996, and currently employs a Coordinator and Care Support Worker.

Goals

To enhance the efficiency of the ONFC to respond to the life long care needs of older Aboriginal people who may have limited mobility related to a persistent medical condition or physical disability, in a manner that is consistent with the mission statement, philosophy and overall goals and objectives of the ONFC

To provide culturally appropriate services that strengthen the physical, spiritual, mental and emotional well-being and enhance the quality of life for those receiving life long care services.

Services for Eligible Clients
(eligibility to be pre-determined through intake process)

  • Adult Day: providing social, recreational and physical activities and/or meals for the frail elderly, transportation provided as available, with assistance of volunteers.
  • Congregate Dining: providing meals and social activities for the frail elderly, with assistance of volunteers.
  • Transportation: providing transportation to medical appointments and for participation in LLC activities for the frail elderly; minimum 24-hour notice required.
  • Home Maintenance and Repair: arranging for heavy housekeeping–seasonal housecleaning, washing indoor walls, drapes, slipcovers, windows, cupboards; paid service assumed by clients.
  • Friendly Visits: visiting, assisting with shopping needs, etc., with assistance of volunteers.
  • Security Checks: checking on the safety and health of isolated elderly clients through weekly visits and/or telephone calls, with assistance of volunteers.
  • Caregiver Volunteer Hospice: supporting clients in palliative care, including bereavement support; service provided by trained volunteers.
  • Aboriginal Support Services: providing referrals, advocacy and information for clients, with assistance of volunteers.
